Do credit cards with a zero balance have to be included in a personal bankruptcy, i.e. will my relationship with any unsecured creditors with whom I have no debt be adversely affected.

When you file for personal bankruptcy you are required to deliver all your credit cards to your trustee. So while the card isn’t directly affected you are not permitted to retain any of these accounts regardless of whether there is money owing on them or not.
